Appointments
If your medical problem is such that you must be seen immediately, every effort will be made to see you in our office.
If you are unable to keep you appointment, please call as far in advance as possible so we my schedule another patient in your place and reschedule you for an appointment that better suits your schedule.
Information Requirements
Prior to, or at the time of your visit, we will ask you to complete a patient information form. It has a bearing on your health and physical status; the physicians need to know your occupation, age, ect. All information is strictly confidential.
You will also be requested to sign an authorization for release of medical information and authorization for your insurance company to pay the doctors directly for covered charges.
Telephone Procedures
Our phone policies are structured to allow us to care for all patients with minimum interruptions, to answer questions, and take care of urgent problems. Calls that are not urgent will be returned at a mutually convenient time.
Prescriptions
During evenings and weekends, it is difficult for the physicians to determine if a prescription is indicated because they may not have access to your medical records. Therefore, prescriptions and refills are handled during our regular office hours only.
Billing
We gladly file insurance claims as a courtesy you our patients. However, all co- payments, co- insurance, and deductibles are due at the time of service.
It is important to know that each patient is ultimately responsible for payment of his or her bill regardless of the status of any insurance claims. Payments made by cash, check, Master Card, or Visa. Cosmetic Procedures are paid by cash, credit card or prearranged financing.
If you financial situation prevents you paying our bill, please let us know so we can make arrangements. Accounts that are not paid within ninety days are generally turned over to our collection agency unless previous arrangements have been agreed to.
Please be prepared to present your insurance card and I.D. at the time of your visit. If your name has changed, please give the receptionist your new card and we will update your information.
If your insurance plan requires a referral from your primary care physician, it is your responsibility to obtain a referral prior to your appointment. Our staff will help you with this process, but if we fail to obtain a referral, you may not be seen by our physicians.
Our physicians participate in many health plans. Please call your insurance company or check with you employer prior to your visit to make sure that the physician you see is a participating provider.
Patient Confidentiality
Montgomery Otolaryngology will make every effort to ensure your medical information remains confidential. It is illegal and unethical for us to discuss your medical condition with anyone or to release medical records to anyone except as provided in our Privacy Policy. We release your records to other physician offices, insurance companies, Medicare and Medicaid if requested. Please ask for a copy of out Privacy Policy when you visit our office.
